That is the distinction worth understanding. Most clinical skincare is built around a single high-percentage active. It forces a visible short-term response and it charges the barrier for it. Where an exosome appears at all, it is usually a skincare company's interpretation of one, specified by a marketing department rather than by a release panel.

Exosomes are membrane-bound extracellular vesicles that carry growth factors, signalling proteins and intercellular communication in a form the skin recognises as its own. The serum applies the same thinking that governs the biologics: work with the signalling the skin already uses rather than overriding it. Everything else in the formula exists to support that layer, not to compete with it.

The four layers

1. Signalling — ≈18 billion exosomes

Human umbilical cord-derived exosomes, from the RENOVÓ clinical line. This is the layer that distinguishes the product and the reason the rest of the formula is restrained: there is no purpose in delivering a signalling fraction and then compromising the barrier it has to work through.

2. Peptides — five systems

Acetyl Hexapeptide-8 at 3%. A matrix peptide complex of Palmitoyl Tripeptide-1, Palmitoyl Tetrapeptide-7 and Palmitoyl Tripeptide-38. Copper Tripeptide-1. sh-Oligopeptide-1. Each does separate work and each is present at a concentration that does it.

3. Active botanicals

Centella asiatica appears as its four isolated actives — madecassoside, asiaticoside, madecassic acid and asiatic acid — rather than as a whole-leaf extract, which is the difference between a specified input and a marketing ingredient. Haematococcus pluvialis and oxyresveratrol carry the antioxidant fraction. Croton lechleri resin, tremella fuciformis polysaccharide and pseudostellaria heterophylla root complete the layer.

4. Hydration and barrier

Niacinamide at 4%, with ectoin, sodium PCA, beta-glucan and acetyl glucosamine. Barrier structure is treated as part of the formulation rather than as an afterthought, because a compromised barrier is what makes aggressive actives feel like they are working.

Cold chain

The serum is held frozen until it ships, then dispatched in an insulated cooler with an ice pack on a two-day service. On arrival it must be refrigerated, and kept refrigerated between uses.

This is not a packaging flourish. A biologically active formulation that is warehoused at room temperature, trucked at room temperature and then left on a bathroom counter is not the same product at week eight that it was at week one. Handling it cold is the cost of the claim, and it is the part of this product most brands in the category quietly skip.

The airless dropper does the same job at the point of use. An open-mouth jar or a conventional pump exposes the biological fraction to air and to the user's fingers between applications.

The serum is supplied to clinical accounts at a flat wholesale price with no minimum order. Suggested retail is set so that a practice dispensing it earns a full hundred per cent markup — the physician-dispensed category standard, and the reason it is worth the shelf space it occupies.

A practice already has refrigeration, which removes the one genuine friction in this product. It is designed to sit alongside a treatment protocol rather than replace one: a patient who has completed an in-clinic protocol leaves with a formulation built on the same clinical material, which keeps the practice present between visits and keeps the margin that would otherwise go to a retail brand.

It is not a drug and it is not a biological product for injection. It has not been evaluated by the Food and Drug Administration.
